Our Team
Simply fill out the health questionnaire, attach payment type for verification, (you are NOT billed at that time), it’s just pre authorization, then the medical questionnaire. Your health information will be reviewed by the Doctor and once approved the weight prescription will be sent to our partner pharmacy for fulfillment right to your door.
